Eat Right Live Right Program

* Provides nutrition education and awareness through one-on-one nutrition counseling sessions, group education, as well as community events such as presentations, expos, and health fairs.
 

Eligibility: Contact physician and ask them to fill out the Eat Right, Live Right Referral Form, making sure the form includes a complete diagnosis and a doctor’s signature.

Application Process: By appointment.

Required Documentation: Referral from doctor.

Fees: For individuals without coverage for nutrition counseling, there is a self-pay option, which can include a 5-month payment plan or be paid in full. Individuals may apply for the financial assistance program that is based on household size and income. Information on the financial assistance program and the application may be found at mvhealthsystem.org/financial-assistance-program.
