(Program)
- Main Street - Provides crisis intervention, advocacy and mentoring to address homelessness, problems at school, poverty, and to prevent involvement with the criminal justice system.
- Intensive Case Management - Offers case management for mental health and assists families in creating individualized plans for the care of their children.
Eligibility: Main Street: Individuals ages 8-21 who are eligible to receive services through the Office for People with Developmental Disabilities (OPWDD) in Herkimer, Madison and Oneida Counties.
Application Process: Call for appointment.
Required Documentation: Proof of address and documentation of medical and/or educational background.
Fees: Fees vary base on services. Call for more information.
